Introduction: What Is Berberine?

Extracted from barberry or Canadian goldenseal, berberine is an alkaloid studied, among other things, for its benefits to metabolic health. It is often compared to a “natural Ozempic.” But what is the reality regarding its mechanism of action? That’s what we’ll demystify in this article to determine when, how, and for whom berberine can be an interesting and relevant natural alternative.

Berberine vs. Ozempic: Why Are They Compared?

In reality, their mechanisms of action are completely different. While semaglutide (Ozempic) mimics a hormone to directly target appetite and satiety in the brain, berberine promotes weight management indirectly by profoundly rebalancing blood sugar and cholesterol metabolism.1

Berberine, Blood Sugar, Insulin Resistance, and Cholesterol: What Does the Research Say?

Berberine can be a real help for people suffering from metabolic syndrome. This condition is characterized by a large waist circumference (due to excess abdominal fat), high blood pressure, resistance to the effects of insulin (insulin resistance) or diabetes, as well as abnormal cholesterol and blood lipid levels (dyslipidemia).2

Berberine helps reduce overall insulin requirements while optimizing insulin recognition by cellular receptors, particularly in muscle tissue. This improved insulin sensitivity allows the body to more effectively take up and metabolize sugar.3

2) Regulation of hepatic sugar and cholesterol metabolism

In the liver, berberine acts as a safeguard: it prevents the liver from producing excess sugar (by blocking neoglucogenesis) and limits the synthesis of new fats (fatty acids).

Meta-analyses confirm that taking berberine, either alone or in combination with silymarin (derived from milk thistle), effectively influences lipid metabolism. This synergy helps reduce LDL (“bad”) cholesterol by 25% and triglycerides by 35%, while increasing “good” HDL cholesterol.4, 5, 6

3) Inhibiting sugar absorption and balancing the gut microbiota

It is in our intestines that berberine first exerts its therapeutic effects, since less than 5% of the substance is actually absorbed into the bloodstream. On the one hand, it has the unique ability to slow the absorption of some sugars by inhibiting alpha-glucosidase, an intestinal enzyme responsible for breaking down complex carbohydrates into simple sugars. By slowing this process, it limits glucose absorption and moderates the rise in blood sugar levels after meals.

On the other hand, it effectively cleanses our gut microbiota by eliminating harmful, inflammation-causing bacteria (LPS) and promoting beneficial bacteria that produce butyrate. This valuable compound improves insulin sensitivity and supports the repair of the intestinal barrier. By improving the balance of the gut microbiota, influencing the genes responsible for fat metabolism, and activating AMPK (a key enzyme for regulating energy and glucose), berberine is thought to significantly reduce BMI and waist circumference, while improving obesity-related health markers such as metabolic syndrome.11

As for its appetite-suppressing effect in humans, it is less pronounced than in animal studies. The reduction in appetite appears to be more subtle and indirect—achieved through the stabilization of blood sugar levels—rather than by acting directly on the brain, as the drug does. By preventing blood sugar “roller coaster” effects (glucose spikes followed by sharp drops), it reduces sugar cravings and false hunger signals.12, 13, 14

The clinical studies reviewed used average doses of 500 mg to 1.5 g per day for 3 months, taken in divided doses.24, 25 According to the scientific literature, the most common side effects (mild gastrointestinal discomfort) generally occur at doses between 600 and 900 mg per day.
